Establish the minimum operating standard

The interim lead will create dependable arrangements across sites while recognising differences in demand and available resources. Review appointment access, opening reliability, patient registration, stock availability and escalation to higher levels of care. Identify which variations are justified by local need and which expose patients to inconsistent service.

Agree a practical operating standard with clinical and local management colleagues. It should define essential responsibilities, information and escalation rather than become a large manual that teams cannot use. Site managers must know which decisions they can make and where clinical or executive approval is required.

Allocate scarce support deliberately

Assess staffing coverage, shared support functions and the time spent on avoidable administrative work. Lead improvements to scheduling and coordination within delegated authority. Where a site cannot sustain the intended service, present options that explain the consequences for access, continuity and cost rather than relying on repeated emergency cover.

The lead will also examine how information moves between locations. Patients should not face unnecessary repetition because records or referral details remain isolated. Technology changes must be proportionate to actual capability, including connectivity and staff training requirements.

The handover test

The permanent organisation should be able to maintain site standards and identify deterioration without the interim lead visiting every location personally. Leave accountable site owners, an escalation process and a small set of meaningful measures. Include unresolved gaps and realistic resource requirements.

Success means more predictable access and service continuity, supported by evidence from patients, staff and operating performance. Decisions on clinical protocols, professional scope and treatment remain with appropriately authorised clinical leaders. Material staffing and investment changes require the relevant executive approvals, while the interim lead is accountable for coordinating and delivering the agreed operational improvements.
