Review the learning process

Strengthen the connection between findings, corrective actions and evidence of improvement. Recommend measures that consider both outcomes and the reliability of relevant processes, with clinical validation. Support proportionate governance so serious concerns receive attention without overwhelming teams with undifferentiated actions.

The review should examine whether staff receive feedback after raising a concern and whether lessons reach other teams facing a similar risk. Action plans should address process design, resources or supervision where those are contributing factors, rather than defaulting to reminders or retraining. The adviser should help define evidence that an improvement works under normal conditions. Where measures change, consider reporting behaviour and data quality before interpreting the movement as better or worse safety, with clinical interpretation supplied by authorised professionals.

Candidate qualifications

Candidates should have healthcare quality, patient-safety systems or clinical-governance improvement experience. They need skill in analysing processes and facilitating constructive learning. A background in operational improvement can be relevant when paired with appropriate clinical expertise for the work required.

The standard of a useful engagement

The provider should gain clearer ownership, better feedback and a way to test whether actions reduced the underlying risk. Candidates should explain a case where an action was completed but the problem persisted. The role is advisory; it does not authorise independent clinical practice, guarantee safety outcomes or replace established incident investigation and professional accountability processes.
